Henry Ford Health System Case Management Associate (Full-tme - Day Shift) - Behavioral Health - Jackson, MI in Jackson, Michigan
The Behavioral Health Associate Case Manager’s responsibility is to provide patient care management services for identified behavioral health patients who are currently involved in an Allegiance Health Behavioral Health program.
The Behavioral Health Case Manager will engage identified behavioral health patients in a collaborative process of identifying, planning, accessing and advocating for, coordinating, monitoringand evaluating resources, supports and services necessary to optimize their functioning and well-being. This includes coordinating services within and between Allegiance Health Behavioral Health programs, providing and coordinating referrals to needed community services and resources and facilitating transitions of care to maximize effectiveness of care and a positive patient experience.
The Behavioral Health Case Manager will function as a core member of a collaborative care team that involves the patient’s primary treatment provider, a consulting psychiatrist and other health providers that may be involved in the patient’s inpatient and outpatient behavioral health care.
Bachelor’s degree required
Experience working with underserved, transient populations.
Experience working with patients who have co-occurring mental health, substance abuse, and physical health problems.
Discharge planning and behavioral health case management required
Exceptional computer and analytical skills required
Knowledge of behavioral health care of adult patients is required;
Ability to establish priorities and coordinate work activities among multiple requests required
Must be capable of standing, walking, stooping, bending and lifting up to 25 pounds.
Must have previously completed or does complete motivational interviewing training within six months of hire.
Bachelors of Social Worker license in the State of Michiganpreferred
Minimum of two years of experience working with behavioral health patients in an ambulatory and/or community health setting preferred
Working knowledge/experience in utilization management, managed care, and payer issues is preferred
