Holland Hospital RN Care Manager - Neurology in Holland, Michigan
The RN is responsible for the provision and coordination of patient centered care. The RN will assist the physician with patient calls, maintaining medical record information, coordinate patient care with other health care providers including necessary prior authorizations. The RN will focus on prevention, health promotion, patient education, and patient assistance. Under the supervision of a physician the RN is involved in rendering medical care to relieve the symptoms of patients' injuries, diseases, and conditions as well as advise, counsel and instruct patients on health care treatment and prevention. As a Care Manager, the RN serves as a patient advocate/liaison, overseeing the continuum of care and chronic disease care management for the patient. The Care Manager will take a proactive role by early intervention and collaborative practice with staff nurses, physicians, ancillary services, payer sources, and outside agencies assuring appropriate use of resources. The Care Manager will focus on positive outcomes and streamlining the patient care delivery process while avoiding duplication and fragmentation of patient care through hands-on care management of patients. Works with payers and gatekeepers to assure necessary pre-certifications / authorizations are obtained. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.
Minimum Education/Training: Bachelor's Degree in Nursing; consideration may be given to experienced candidates actively pursuing a Bachelor's Degree in Nursing. Consideration may be given to candidates with these certifications: Certified Diabetic Educator (CDE); or Certified Asthma Educator (CAE). Minimum of three years experience in clinical practice with a focus on education, quality improvement processes/programs, and regulatory agency compliance. Clinical experience in a patient care setting. Care Management experience. Formal Training in Case / Care Management or must complete required training within 60 days of hire. Training may be required by certain payers as part of pilot programs.
Registration/Certification or License Required: RN licensure in the State of Michigan. Current BLS for the Healthcare Provider Certification (or completed within 60 days of hire).
Preferred Education: Accredited Care Manager.
Preferred Experience: Five years experience in clinical practice with a focus on education, quality improvement processes/programs and regulatory agency compliance.
Physical Requirements: Frequent sitting. Occasional standing, walking. Lift 10 lbs. box overhead. Lift and carry 15 lbs. Push/pull 20 lbs. Occasional twisting, bending, reaching, squatting, kneeling. Occasional keyboard use. Frequent talking, hearing.
Other Requirements: Effective communication skills. Ability to work well under stress within a changing environment. Crisis Management and conflict resolution skills. Strong working familiarity in working with MS Excel, Access, Word applications.
