If you would like to help us achieve our mission, please apply for the following opportunity.DescriptionLutheran Social Services of WI and Upper MI is currently looking for a part-time, 18 hour per week, HUD Service Coordinator to join our team. The Service Coordinator HUD provides information and referrals to community connections to tenants following guidelines, as defined in the HUD Housing Handbook Section 202, who need supportive community services to maintain self-sufficiency. This position will cover properties in Marquette, MI and Harvey, MI. Travel and mileage is compensated for time in Marquette.Essential Duties and responsibilities:This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.and#8226;Provide general informal case management, including intake, and referral services to all tenants needing such assistance when from case management is not available.and#8226;Assessment of health, psychological and social needs. Development of an individually tailored case plan for services and periodic reassessment of the tenantand#8217;s situation.and#8226;Establishes linkages with all agencies and service providers in the community: shops around to determine/develop the best and#8220;dealsand#8221; in service pricing to assure individualized, flexible and creative services for the involved tenant(s).and#8226;Sets up a directory of providers for use by both project staff and tenants.and#8226;Refers and links the tenants of the project to service providers in the general community; these are for example, case management, personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurse, preventative health screening/wellness and legal advocacy.and#8226;Educates tenants on service availability, application procedures, client rights, etc. providing advocacy as appropriate.and#8226;May develop case plans in coordination with assessment services in community.and#8226;Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.and#8226;May set up volunteer support programs with service organization in the community.and#8226;Helps other tenants build informal support networks with other residents, family and friends.and#8226;May provide training to project tenants in the obligation of tenancy or coordinate such training.and#8226;May educate other staff on the management team on issues relating to aging in place and service coordination, to help them better work with and assist the tenants.and#8226;Presents workshops or informative sessions to tenants on subject of tenant interest.and#8226;Demonstrate the ability to work with tenants, the agency and the community in a positive manner as well as the ability to work independently.and#8226;Performs other duties as assigned.
