Munson Medical Center Registered Nurse-Service Coordinator - Home Care in Grayling, Michigan

ENTRY REQUIREMENTS

Require graduation from a recognized school of nursing and current Michigan RN licensure. BSN preferred.

Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health prior to January 1, 2009, are encouraged to obtain a BSN. Certification in the specialty area of practice is also encouraged.

Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health on or after January 1, 2009, are required to obtain a BSN within ten years of their system hire date. Certification in the specialty area of practice is encouraged.

5 years of nursing experience with 2 years of acute care required. Home Care or Hospice experience preferred.

Expert health assessment, organizational and interpersonal skills.

Proven self-directed professional with ability to make independent judgments directing patient care.

Demonstrated effectiveness in working with others, with excellent oral and written communication skills and human relations skills.

Meets MHH established requirements for proof of health status. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or transfer assistive devices to avoid manipulating more than 35 pounds.

Possesses current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices under various weather conditions.

Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.

SPECIFIC DUTIES

  5. Communicates openly and effectively with clients, family members and members of the health care team to ensure quality care for the client/family.

  6. Serves as liaison between physician, referral sources, and agency personnel in assessing appropriateness of patient referral and determining ongoing patient care needs.

  7. Assesses and reviews new patient referrals, assists with problems related to staffing, and assures continuity of care in the home setting.

  8. Schedules new admissions, resumptions of care, and routine visits for home care professional and paraprofessional staff.

  9. Participates in the performance improvement process under the direction of the manager or director by being involved on teams, collecting data, or preparing reports as appropriate.

  10. Participates in the orientation of the field staff.

  11. Notifies the manager of critical patient, customer, or staff situations promptly.

  12. Serves as an educator and resource person to staff.

  13. Complies with MHH department policies, procedures and guidelines.

  14. Complies with standard precautions, infection control and safety procedures.

  15. Meet specific duties of Registered Nurse job description to be able to make patient visits when necessary.

    1. Employees with e-mail are required to maintain proficiency in the basic functions of the program and are also required to regularly check email and keep calendars up to date.
  16. Performs other duties and responsibilities as assigned.
