Henry Ford Health System Transfer RN- Utilization Review- Full Time/36 HRS- Midnights-Detroit, MI in Detroit, Michigan

The Transfer Nurse is a Registered Nurse who facilitates the transfer of patients to Henry Ford Hospital in a safe, successful and efficient manner. This position is the first point of contact for referring entities (e.g., physicians, hospitals, other care providers) seeking to refer, consult and/or transfer patients to Henry Ford Hospital from across the region and Canada. Coordinates all aspects of patient transfers. Utilizing clinical expertise prioritizes patient transfers based on urgency/acuity, mode of transportation, ground or air, transfer care, receiving preparation/communications, bed management, quality assessment, etc. This position is also responsible for evaluating and coordinating the flow process involved in taking calls and facilitating the acceptance and triage of calls throughout the HFH transfer center. Will use approved clinical screening criteria reviews and screens all proposed and actual admissions for appropriateness of services and setting, recommending if the patient is a candidate for observation or inpatient status. Under minimal supervision, coordinates the placement of all admissions (elective, urgent, emergency) and transfers in accordance with clinical standards and guidelines. Coordinates bed utilization across all hospital patient care units, Emergency Department, and all internal and external points of entry. Skills needed for the position include: ability to project a friendly, helpful image on each incoming call, answer calls quickly, responding efficiently and calmly during emergency situations, strong clinical assessment skills, working knowledge of applicable regulatory rules and requirements, ability to ask questions and provide disposition recommendation, problem solving skills, ability to work effectively and collaboratively with diverse members of the health care team, ability to communicate effectively and concisely and use clinical and organizational skills to assist Henry Ford Hospital to grow market share and enhance/maintain reputation in outlying communities.

EDUCATION/EXPERIENCE REQUIRED: Bachelors Degree in Nursing. Minimum of three to five years of clinical experience required. Work independently using the professional skills. Experience in acute care or appropriate similar setting, Work independently using the professional skills. Demonstrated ability to work in high-stress, high-acuity setting, to adapt to rapidly changing work environment and to direct and control multiple critical communication events simultaneously. Computer skills to include: word processing and data entry. Ability to manage multi-line telephone simultaneously. Ability to effectively communicate clinical information and interact effectively with physicians and staff to achieve quality outcomes. Time management. Organization and delegation. Negotiation and conflict resolution.

CERTIFICATIONS/LICENSURES REQUIRED: Registered Nurse with a valid, unrestricted State of Michigan license.
