McLaren Health Care Rehab Liaison RN in Bay City, Michigan

Rehab Liaison RN works in coordination with the admissions team including the
appropriate physicians to: coordinate efforts within the admissions office to
ensure timely response to acute care hospital referrals, screen patients for
appropriateness of admission to inpatient rehabilitation, communicate with the
admitting and consulting physicians, effectively speak with patients and family
members throughout the evaluation process, as appropriate, facilitate admissions
to In-Patient Rehab including required pre-authorization and pre-admission
documentation, work with partners toward the goal of problem solving and facilitating
referrals to the appropriate level of rehabilitation.
Essential Functions and Responsibilities:
Analyzes referral patterns of physicians andprioritizes time appropriately to increase business.
Develops relationships with current and potentialreferral sources including insurance case managers and payer sources.
With collaboration from the Rehab Management Team,develops strategic initiatives for market development in assigned territory.
Follows cases throughout the assigned territory byrounding on appropriate call points with goal of incremental referraldevelopment.
Targets and builds relationships with case managers,both internal and external to the organization.
Provides reporting trends and activity to the Rehab ManagementTeam.
For those referrals that are appropriate for admissionto the Inpatient Rehab Unit: In collaboration with the acute carehospital staff, collects appropriate patient information and completes all documentationthat is required for admission. Provides patient and family educationrelated to inpatient rehabilitation. Collaborates with the referringhospital staff, inpatient staff and admitting physician to determine anadmission plan, and to set a date and time that is appropriate for the patientand meets the needs of the referring hospital.
Completes the pre-admission screen and ensures thatappropriate physician review is completed prior to admission.
Develops and maintains effective working relationshipswith all referral sources. Facilitates professional communication witheach employee of the referring hospital.
Encourages, coaches, and monitors teamwork and directcommunication with co-workers.
State licensure asRegistered Nurse, Physical Therapist, Occupational Therapist or Speech LanguagePathologist.
Associates degree inlicensure discipline.
2 years’experience in business, management or allied health.
Bachelor’s degreein licensure discipline.
Within 12 months,attains, and thereafter maintains CRRN credentialing or other advanced credentialingin related health care field.
